  2. What medical conditions have you been diagnosed with?

  3. What treatment are you getting right now?

  4. What symptoms do you experience?

  5. What type of work have you done in the past?

  6. How much money do you make?

You don't need to have anything ready for the call, but the more details you can share about your diagnoses, work history, and where your case stands, the better we can help.
